ResponsibilitiesProvide a high level of customer service

to residents and Team Members by achieving the standards outlined in the MG CARES program.

Overall 24/7 accountability for clinical care coordination for assisted living residents, ensuring all programs and services are delivered according to the philosophy and standards of Merrill Gardens.

Resident Services:

Develop and implement the highest standards for assisted living services.

Work directly with residents to evaluate needs and services.

Conduct and/or assist with electronic evaluation of incoming residents, and with periodic re-evaluation, to ensure services rendered are documented and charged for according to Merrill Gardens’ policies.

Implement services during new resident move-in; help resident with orientation to and adjustment to the community.

Provide direct care to residents as needed according to their identified service needs.

Accurately assess, communicate, and document resident status. Observe and report any significant changes in resident behavior and health to the GM, physician, and family per state regulations.

Conduct regular meetings with residents and their families to ensure their needs are being met.

Communicate with resident family members, physicians, and pharmacists as needed regarding residents and services provided.

Ensure all Resident Rights and Responsibilities are protected and enforced. Recognize and assist in the prevention of elder abuse, neglect, and exploitation and report to the appropriate sources per state regulations.

Address difficult resident behaviors with dignity and respect.

Guarantee compliance with all State Rules and Regulations of assisted living.

Supervisory Responsibilities:

Provide team members with coaching, feedback, and developmental opportunities.

Challenge and inspire team members to achieve results.

Evaluate team members’ skills and competencies and ensure the completion of performance reviews.

Set clear expectations and hold team members accountable for performance.

Oversee training and development of team members directly and indirectly managed; ensure training and certification of all AL staff is completed as required by Merrill Gardens and state regulations.

Provide coaching, direction, and leadership support to team members to achieve employee engagement and customer delight.

Mentor team members using electronic care system.

Build effective teams by determining adequate staffing levels, identifying and recruiting top talent.

Prepare staffing schedules to meet all community requirements for standards of care.

Administration:

Adhere to budgetary guidelines established by Merrill Gardens.

Coordinate with General Manager for financial stability of assisted living department from both census development and expense control.

Provide General Manager with weekly status reports on all AL residents.

Maintain all resident records and department documentation.

Comply with state-mandated reporting as required.

Audit AL services for regulatory compliance.

Provide additional management reports as required.

Minimum RequirementsEducation:

Current LPN/LVN or RN licensure, as specified by state agency, preferred.

Bachelor’s Degree or equivalent.

First Aid/CPR certification within 30 days of hire where required by state.

Completion of dementia training as required by state regulations within six months of hire date if applicable.

Experience:

3 years’ related experience, training, or equivalent combination of education and experience.

Experience in assisted living, geriatric nursing, or gerontology and dementia care.

Experience in medication administration and hands-on personal care preferred.

Demonstrated ability in budgeting and cost control.

Experience in operating an electronic care system.
