Full-timeDepartment: ANIMOBackgroundSince 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice and throughout Los Angeles County. The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through 20+ programs in four pillars of service: Outreach and Engagement, Housing Services, Mental Health, and Education and Vocational Training. St. Joseph Center serves more than 10,000 people each year.Brief Description of Principal ActivitiesMultifaceted intervention, prevention, mental health, and housing assistance is offered through street outreach and 9 service sites throughout Los Angeles County. St. Joseph Center also provides on-site services at five permanent supportive housing developments, and will provide services at an additional six more sites in the coming year. St. Joseph Center’s total holistic approach to services provide our clients with the help they need to regain their stability.Job SummaryThe Case Manager serves as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality care in accordance with the rules and regulations established by the Los Angeles Department of Mental Health for contracted agencies while adhering to the mission of St. Joseph Center.ESSENTIAL DUTIES & RESPONSIBILITIESThe Case Manager’s key areas of responsibility include:Provide targeted outreach, engagement, and education activities to Latino individuals and families in order to increase awareness regarding overall mental health and health issues.Perform routine case management activities while working with other program staff providing services to clients.Help prospective clients gain access to needed community resources and support.Assist in establishing services as defined by agency and funding sources.In conjunction with other staff, coordinate activities, recruit participants, arrange event logistics, and provide staff support.Facilitate therapeutic groups, including working alongside other staff to develop and implement curriculums.Assist and explore individual rehab sessions by providing coping skills to help support clients with mental well-being.Participate in regular Latino ISM team meetings.Document all advocacy and case management activities as required by agency and/or funders.Other duties and responsibilities may be assigned. The duties and responsibilities listed are designed to provide typical examples of the work performed; not all duties and responsibilities assigned are included here, nor is it expected that all similar positions will be assigned every duty and responsibility.QUALIFICATIONSKnowledge, Skills & Abilities:Fluency in Spanish/English language required.Interest in immigration issues and/or family systems is a plus.Must be highly motivated and a self-starter.Must have the ability to communicate with and relate to a diverse group of people including clients, community, and other staff.Must have excellent administrative and organizational skills.Experience:Experience providing mental health and substance use case management services in a community setting with at-risk populations; experience in Hispanic culture is preferred.Education:Bachelor’s degree from an accredited college or university in social work, human services or a related field.Direct Reports:This position has no direct supervisory responsibilities.Certificates, Licenses and Registrations:Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.Valid automobile liability insurance.Travel Requirements:Travel to meet clients may be required.Transport clients using company vehicle as needed.The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
