Develops patient data base through a comprehensive health history, physical examination and laboratory studies/diagnostic tests as indicated. Orders diagnostic tests and interprets data, discriminating between normal/abnormal findings. Formulates, implements and evaluates an ongoing plan of care incorporating interdisciplinary input. Manages treatment modalities. Counsels and supports patients and families, utilizing appropriate knowledge, training and experience. Facilitates/provides opportunity for improvement in outcomes for service line patient population. Performs interventions per approved standardized procedures.DocumentationCreates/Maintains appropriate documentation and record keeping for all patient contacts.EducationAssists in developing learning experiences to meet the needs of staff, community, and patients. Provides health teaching to staff, patients and families to increase their knowledge base. Enhances patient / family learning and health behaviors. Demonstrates critical thinking and problem-solving skills in area of specialty when consulted by interdisciplinary team members.LeadershipPromotes interdisciplinary planning and coordination of ongoing patient care. Collaborates and refers appropriately to other members of the health care team. Acts as a role model for staff. Functions as a contributing member within the appropriate service line.
