Job Title: Oncology Nurse NavigatorThe Oncology Nurse Navigator provides expert individualized care coordination to patients and their families, ensuring a timely schedule for diagnostic work-up, results reporting, and communication between the patient and the multidisciplinary team, ensuring the patient receives necessary referrals and follow-up. The Oncology Nurse Navigator serves as an advocate for the patient, promoting timely, competent, compassionate care and enhancing the physician/patient relationship, improving the experience of the patient with cancer.Responsibilities:Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.Collaborates and continuously partners with chronically ill or high-risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach.Evaluates the needs of cancer patients and educates them about their cancer diagnosis.Prepares patients and caregivers for what to expect throughout the cancer care continuum.Creates and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist, and family/caregiver(s).Provides clinical support to Patient Navigators and Sidney Kimmel Cancer Center (SKCC) patients.Educates patients and families about disease processes and potential side effects.Assists patients with treatment decision-making; develops and uses decision aids as appropriate.Attends and supports Tumor Boards.Reduces emergency room utilization and hospital readmissions through outpatient symptom management.Follows patients throughout the care continuum, including inpatient admissions in collaboration with inpatient care management resources.Supports transitions of care and coordination between SKCC sites.Provides clinical consultation to the Patient Navigators.Educates multidisciplinary colleagues about the role of the Nurse and Patient Navigators, identified barriers, available resources, and the navigation process.Supports program growth and patient experience in all patient interactions.Education:Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred.Experience:Three (3) years of Healthcare/Medical experience in Oncology/Cancer Care preferred.Additional Information:Covid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson's clinical entities or at the University.
