OVERVIEW:

The Drop-In Center Program Lead Case Manager will provide case management and supportive services for children and families, adults, and older adults. This role requires someone adaptable and self-reflective, as well as an empathetic team player with strong interpersonal skills and a desire to support the needs of underserved and underprivileged adolescents and adults from diverse backgrounds.

The Behavioral Health Division serves clients under multiple programs:

Supportive Therapeutic Options Program (STOP)

CalWorks

Prevention and Early Intervention-school based (PEI)

Older Adult Outpatient Program

Child-Parent Psychotherapy (CPP)

Community-based Drop-In Center

Substance Use Treatment Alternative Activities

Individual Placement Services

POSITION SUMMARY:

The Drop-in Center Lead will manage the team's case for creating and facilitating groups for community members in Santa Clara County.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Assume overall responsibility for the day-to-day lead of the Drop-In Center services for adolescents, adults, and older adults to ensure quality service delivery.

Facilitate and co-facilitate drop-in services groups in the community.

Assess and direct continuous quality assurance/improvement to ensure quality and effectiveness of service delivery, including quality planning and strategies, compliance, and corrective-action management experiences.

Review and approve the psychoeducational, recreational, and social skills curriculum to determine the most appropriate presentations.

Oversee and ensure timely data entry; provide statistics to Santa Clara County program contract monitor every month.

Communicate and collaborate with the Program Manager, Senior Clinical and Wellness Division Director, and other department managers to ensure effective interdepartmental workflow strategies.

Participate in recruitment, training, supervision, and evaluation of case managers to ensure the success, sustainability, resourcefulness, and ongoing development of program services, support, healing methods, and evidence-based practices to increase clients' social and emotional well-being.

Participate in annual staff performance evaluations and goal-setting, assess staff needs, and schedule training accordingly.

Provide strong leadership and people management that reflects Catholic Charities' values, including mutual respect, compassion, and integrity.

Ensure that all program contract requirements are met and regular contact with the appropriate county contacts is made.

Attend all collaborative county/city and other program meetings.

In collaboration with the Program Manager, develop and implement new supportive programming.

Other duties as assigned by the supervisor.

SUMMARY OF OTHER DUTIES:

Administrative/Quality Assurance

Conduct and ensure program evaluation and upkeep of records and database functionality to support programs.

Maintain, update, and establish documentation requirements in collaboration with the Data Systems Coordinator.

In partnership with the Senior Director of the Clinical and Wellness Division, collect accurate program statistics and ensure timely submission of contract reports.

Consistently review and update training materials and division protocols aligned with strategic planning, program development, and evaluation.

Stay abreast of developments and trends in child development and school readiness, case planning, and therapeutic interventions for trauma impacts on older adults and adolescents.

Coordinate effectively with the senior director of clinical services, communicate effectively with community partners, and respond to client concerns regarding service delivery.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE

Bachelor’s degree in social work, human services, counseling preferred, or equivalent combination of education and experience.

Minimum of two (2) years experience in human services or Behavioral Health.

Experience in office and administrative processes, procedures, and program development.

Experience in training staff and volunteers is preferred.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

Bilingual in Spanish/Vietnamese/Mandarin preferred.

Knowledge of community resources, organizations, and supports.

Knowledge of mental health and alcohol, substance abuse, and recovery treatment services.

Good oral and written skills.

OTHER QUALIFICATIONS:

Criminal background check via Livescan fingerprint.

Must have TB test performed and submit results.

Provide proof of COVID-19 vaccination and booster status.

Automobile, valid driver’s license and auto insurance per agency policy; or access to reliable transportation to meet with clients if necessary.

WORKING CONDITIONS:

Willing to go to clients or community agencies as necessary through own vehicle or public transport.

Some travel within Santa Clara County for client services. Some travel between sites for program needs.

HOURS AND OTHER CONDITIONS:

Standard office hours are Monday through Friday, from 8:00 a.m. to 5:00 p.m.

This is a full-time, non-exempt position. Availability to work flexible hours may be needed based on client services (i.e. working evenings until 7:00 pm, and/or working weekends).

MANDATED REPORTER STATUS

This position requires you to work with adults and children, and your profession qualifies you as "mandated reporters" of Elder abuse or neglect. Child abuse or neglect, which is found in California Penal Code Section 11164-11174.3 and Welfare and Institution Code 15610-15610.70. In addition, you are required to report suspected cases of child abuse and neglect and Elder abuse and neglect.
