The Nurse Director Case Management will report directly to the Clinical Operations Administrator. In this crucial role, the Nurse Director Case Management will be instrumental in optimizing patient care, resource utilization, and financial outcomes, ensuring our facility remains at the forefront of healthcare delivery.Key Responsibilities
The Nurse Director Case Management will:Oversee and manage the Case Management department, including utilization review, discharge planning, and care coordinationDevelop and implement strategies to improve patient outcomes and reduce length of stayCollaborate with multidisciplinary teams to ensure seamless care transitionsMonitor and analyze key performance indicators to drive continuous improvementEnsure compliance with regulatory requirements and accreditation standardsThe Nurse Director Case Management will work closely with a diverse group of healthcare professionals. Day-to-day interactions will include collaborating with physicians, specialists, nurses, social workers, and administrative staff. The Case Management Nurse Director will lead a team of case managers, fostering a culture of excellence and continuous improvement. The Nurse Director Case Management also regularly engages with patients and their families, insurance providers, and community resources to ensure comprehensive care coordination.
