The integrated psychiatric nurse practitioner is responsible for supporting integrated behavioral health care provided in primary care settings by a team comprised of physicians, nurse practitioners, physician assistants, and integrated behavioral health providers. This integrated psychiatric nurse practitioner is responsible for performing psychiatric assessments and diagnostic evaluations, initiating and monitoring medications, and coordinating the care plans of patients with complex psychiatric needs with the primary care teams. Brief, evidenced-based pharmacologic services are provided to children, adolescents, adults, and/or older adults in the primary care setting.This integrated psychiatric nurse practitioner is in a consultant role with integrated behavioral health clinicians and primary care teams to develop and implement motivationally appropriate treatment plans for patients with comorbid physical health, mental health, and/or substance use issues. The integrated psychiatric nurse practitioner responds to and provides crisis intervention services when needed. He or she also provides education and training to the IBH and primary care teams regarding behavioral health conditions and their interactions with physical health. They ensure that behavioral health services are delivered in a culturally appropriate fashion aligned with the policies, procedures, and philosophy of PCHC, as well as all state and federal regulations.Essential Duties & Responsibilities for Integrated PNP Crossroads:Provide short-term psychiatric evaluation (averaging 3-6 visits per care episode) and evidenced-based pharmacologic treatment of patients, focusing on patients with diagnostic or therapeutic challenges who are identified in discussion with the patient’s IBH clinician and primary care provider. Crossroads’ site has a primarily adult homeless focus with episodes of care that could be lengthier than the average rate described above.Functional schedule based upon 30-minute appointment slots in a primary care exam room at the identified site. Crossroads’ site, due to its unique service population, is not limited to the number of visits with patients and can meet with patients as many times as needed across a mutually agreed-upon timeline at various locations in the community.Provides consultation to clinical staff and primary care providers as needed regarding the patient’s care, treatment recommendations, and progress via documentation in the client’s record, face-to-face or telephonic consultation.Coordinated Care/Patient Centered Medical Care Management:

Works with all members of the Health Care Team towards achieving coordinated patient-centered care.Qualifications:A candidate for this position must have a Master’s Degree in Nursing as an Advanced Practice Nurse specializing in psychiatry.Must be licensed to practice Advanced Practice Nursing in Rhode Island or able to obtain a license.Maintain ongoing professional education as mandated by state regulation. Maintain appropriate hospital privileges in accordance with PCHC policies and the agency’s scope of service.Possession of, or the ability to obtain, a Drug Enforcement Agency (DEA) number to furnish controlled substances.PCHC is EOE/M/F/D/V/SO
