Downtown San Diego areaPosition SummaryUnder the direction of the Program Manager of Unhoused Care, the Case Manager plays a pivotal role in our mission by assisting homeless individuals and families in finding and securing stable housing. This position requires a compassionate, resourceful, and highly organized individual who can connect clients with available housing options, navigate complex housing systems, and provide ongoing support throughout the housing placement process. The Case Manager will work closely with clients to identify their unique needs and barriers, develop personalized housing plans, and advocate on their behalf to access appropriate services and resources.Essential Job Functions & Duties (85%)Conduct comprehensive assessments of homeless individuals and families to determine their housing needs, preferences, and eligibility for housing programsIdentify and access available housing resources, including shelters, transitional housing, and permanent housing options, and assist clients in finding suitable and affordable housing.Guide clients through the application and placement process for housing programs, ensuring all necessary documentation is submitted accurately and in a timely manner.Develop and implement individualized housing plans for clients, addressing barriers such as income, mental health, substance use, and legal issues.Advocate on behalf of clients to access supportive services, benefits, and community resources necessary for housing stability.Provide clients with information and education about their housing rights, responsibilities, and budgeting skills to promote long-term housing stability.Prepare and submit regular reports on client progress, program outcomes, and housing trends to support program evaluation and funding requirements.Collaborate with other service providers, community agencies, and landlords to expand housing options and support services for homeless individuals.Maintain accurate and confidential client records, progress notes, and documentation of housing placements and outcomes.Perform other tasks as assigned by departmentAdministrative Management (15%)Adhere to all local, state, and federal guidelines and laws. Duties, responsibilities and activities may change at any time with or without notice.**Required Qualifications, Skills, and AbilitiesBachelor's degree in social work, psychology, human services, or a related fieldPrevious experience working with homeless populations or in housing advocacy requiredKnowledge of local housing resources, housing laws, and social services.Strong communication, interpersonal, and problem-solving skills.Ability to work independently and as part of a team.Compassionate and nonjudgmental approach to working with homeless individuals and families.Familiarity with data management and case management software is a plus.Basic knowledge of Microsoft Word, Excel, PowerPoint, and OutlookAbility to read and write in English (bilingual or multilingual preferred )Valid Driver’s License requiredCore AttributesProactiveFlexiblePhysical RequirementsProlonged periods of sitting at a desk and working on a computer.Must be able to lift 15 pounds at times.Must be able to access and navigate each department at the organization’s facilities.Total Rewards PackageThis is a full-time non- exempt (hourly) position. $27.66 per hour . This position is contingent on successfully passing a background check, drug screen, and physical.
