This is a part-time position working 32 hours in Jamaica Plain, Dorchester, and Roxbury.Job SummaryProvide occupational therapy services to patients in their place of residence who have impairments, cognitive and/or physical resulting from injury, disease, disability or other causes. Collaborate with the patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs.Key Activities• Develop a plan of care that is patient and family focused and instituting standardized interventions for goal-oriented outcomes.• Daily documentation in a computerized system accurately reflects objectivity and measurability.• Application of standard treatment procedures; Teaching is provided to patients and caregivers.• Interdisciplinary communication occurs regularly.Qualifications and Experience• Two years of experience in acute care, rehab, or homecare as an Occupational Therapist.• Registered with the Board of Registration in Massachusetts.• Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred.• Strong customer service skills and ability to work with diverse populations.• Excellent verbal, written, and electronic communications skills.• Ability to prioritize work and adapt to changing situations.• Travel required within Mass General Brigham Healthcare at Home's geographic area.Mass General Brigham Home Care is an Equal Opportunity Employer.
