Service Coordinator (Mental Health Case Manager)

Job Duties:

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Qualifications:

Prefer a bachelor's degree in psychology, social work, or a field related to mental health.Two (2) years of full-time experience or part-time equivalent in a mental health setting.

About the Campus:

Each client on our campus, whether they speak English, Spanish or Vietnamese, is given an individual rehabilitation plan, empowering and motivating them to be active participants in their recovery. Our personalized recovery plans help clients develop skills to allow them to achieve a more independent, productive and satisfying lifestyle. The campus is right off the 280 (near 880), provides ample free parking for staff, and has many nearby restaurants and parks (including the Los Gatos Creek Trail).

The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.
