Major Responsibilities:Collaborates with health care team to identify alternative placements for patients.Interfaces with members of the health care team, patients, families, and vendors, while making referrals and transfer arrangements.Extracts supplemental information from the medical record in order to make appropriate referrals to facilities.Documents progress of the discharge planning process in the medical record and in the MSM system.Completes cases in the MSM after discharge.Assists with completion of pertinent screenings and contacts appropriate regulatory agencies.Establishes and maintains effective working relationships with facility representatives through telephone and/or direct contact. Maintains a supply of all necessary forms related to the discharge process on inpatient units.Facilitates referrals by telephone and/or fax to rehabilitation, skilled-nursing, or home care facilities. Arranges for durable medical equipment (DME) as directed by care coordination team.Identifies and recommends solutions regarding systems discharge issues.Standard Staffing Level Responsibilities:Complies with established departmental policies, procedures, and objectives.Attends a variety of meetings, conferences, seminars as required or directed.Demonstrates use of Quality Improvement in daily operations.Complies with all health and safety regulations and requirements.Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.Maintains regular, reliable, and predictable attendance.Performs other similar and related duties as required or directed.All responsibilities are essential job functions.II. Position Qualifications:License/Certification/Education:Required:1. Bachelor's degree in health services administration or related field.Experience/Skills:Required:1. Excellent communication, collaboration, and organizational skills. Knowledge of a variety of computer-based software programs such as Word, Excel, etc.Preferred:1. One to three years of experience in health care preferred.Unless certification, licensure, or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.III. Physical Demands and Environmental Conditions:Work is considered sedentary.
