is a nonprofit, 501(c)(3) organization founded in 1968 to enrich the lives of youth, seniors, homeless populations, economically disadvantaged families, and under-served individuals in South Los Angeles by providing and facilitating a variety of essential educational services, social services, cultural activities, vocational opportunities and economic empowerment opportunities.Job Summary:The Case Manager will work with individual YouthSource Center clients to conduct in-depth case management needs assessments that identify individual client needs and remove barriers to client employment, health, and overall well-being. The Case Manager will identify resources and services to address those needs and support client access to services by developing a personalized case plan in conjunction with the client that meets the goals and objectives of the client. The Case Manager will also track client progress and matriculation through YouthSource Center programming and developmental activities.Supervisory Responsibilities:This position has no direct supervisory responsibilities.Duties/Responsibilities:Oversee implementation of services for each clientAssess client needsComplete case plans and other documentation using requisite documentation and following established protocolProvide and maintain appropriate client data and filesParticipate in staff meetings, trainings, and other programs as requestedAssist in program evaluationHandle crisis and/or emergency situations as neededProvide transportation vouchers and other supportive servicesParticipate in all appropriate sessions and meetingsAssist in the development and coordination of special assignments and programsMaintain a professional attitude, confidentiality, and respect cultural differencesComplete all other duties as assignedWork closely with the Job Development staff in keeping open communication and interaction to meet customer satisfaction and job placement goals.Must be available to work some evenings and weekends, if necessary.Required Skills/Abilities:Excellent communication and organizational skills; strong written and interpersonal skillsExperience working with individuals with diverse ethnic backgroundsDemonstrated ability to work effectively as a member of a teamLeadership experience working with community social service and health organizations strongly preferredBilingual/Spanish a plusExperience working with high-risk youth or out-of-school youthEducation and Experience:Bachelor's degree in Social Sciences or similar field (MSW preferred)Three years’ experience providing case management servicesTwo years working in a non-profit organization or similar environmentProficient in Microsoft Office (Word, Excel, Access, PowerPoint)Must have a valid driver license, clean driving record and own personal transportationPhysical Requirements:Must be physically able to operate a variety of equipment including computers, projectors, scanners, copiers, facsimile machines, calculators, etc.Must be physically able to operate a motor vehicleMust be able to exert up to 20 pounds of force occasionally, and/or a negligible amount of force constantly to lift, carry, push, and pull or otherwise move objects, including the human bodyLight Work usually requires walking or standing to a significant degreeRequires the ability to speak and/or signal people to convey or exchange informationRequires the ability to read a variety of correspondence, reports, forms, applications, etc.Requires the ability to record and deliver information, to explain procedures, to follow oral and written instructionsRequires the ability to utilize mathematical formulas; to add and subtract; multiply and divideMust be available to work Monday - Friday 9:00am – 5:30pm and some weekends or evenings may be necessary.Salary:

$55,000 - $67,000 a year, based on experience and qualifications.Only applicants looking to do some real work with youth and not just a job should apply.SPECIAL REQUIREMENTS:Must possess a valid California Driver's License. Due to our in-person and in-office work with LAUSD, City of LA, County of LA, and other work partners that require our team to be vaccinated, we require that all potential candidates must be COVID-19 vaccinated with either Pfizer, Moderna, or Johnson & Johnson.WORKING CONDITIONS:The successful candidate will be required to report to and work from the office complex at least five days a week. Offices are traditionally equipped with computers, telephones, desks, chairs, and office machines. Additionally, each office offers an instructional classroom from which orientations and trainings are conducted.BENEFITS:Excellent full benefits package.SCHEDULE:Must be available to work Monday- Friday from 9:00am – 5:30pm. There may be occurrences when a work or community event occurs on Saturday and/or Sunday. Your work schedule will be flexed to accommodate this change and you will be notified in advance.
