People Incorporated Mental Health Services

is seeking a Mental Health Practitioner to join our Access and Recovery Center (ARC) Housing First program. Access and Recovery Center Housing First is a wraparound service integrating ARMHS, Behavioral Health Home and/or GRH services with clients. This position is responsible for providing client engagement and direct client services and interventions through ARC Housing First. It provides Housing Stabilization and Housing Support to long-term homeless individuals in scattered-site housing programs in Hennepin and Anoka Counties. Supports will be provided through the recruitment and managing of relationships of community housing providers and landlords, while focusing on housing search and placement based on specific needs of individuals. This is executed by exercising a person-centered approach to client support and services, performing assessments, providing education, promoting healthy lifestyles, facilitating individual and/or group meetings, clear and timely documentation and delivering individualized plan of care consistent with client goals and objectives.

Full-Time, Monday- Friday 8:00am - 4:30pm, flexible start & end times

Hiring Range:

$21.60 - $24.00/Hourly, based on qualificationsHiring Bonus:

Principal Accountabilities/Essential Duties:Using person-centered principles, actively engage clients in their mental health recovery, and develop and maintain appropriate relationships that support the client in meeting their individual goals.Facilitate and conduct individual services that address a variety of needs and health concerns for the client. May facilitate and conduct group interventions, if applicable.Document interactions and interventions with clients, according to program requirements, into the organization's Electronic Health Record (EHR) and/or record-keeping system.Provide crisis intervention and de-escalation for clients experiencing mental health symptoms or other emotional and/or behavioral responses while prioritizing client and program safety.Participate in coordination of services through the recognition of the multiple needs of the client, with other providers in the community, in an effort to provide necessary resources to the client.Provide various skill-building, teaching and/or coaching, as it relates to the individual's goal and/or treatment plan.Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.Maintain regular and active contact with clients ensuring services sought are utilized and partnering with the client to determine effectiveness of the plan and next steps.Interact with other team members to provide comprehensive and timely assistance in resolving client issues, sharing information and engagement efforts.Maintain accurate and timely case notes on all client contacts and document all activities tied to the client.Cultivate relationships and provide outreach with realtors, landlords, housing developers and other housing providers willing to provide housing for participants and assist with administration of rental assistance.Help the individual served meet and build a relationship with a prospective landlord and negotiate a lease as needed.Partner with administrative staff to maintain an accurate monthly budget.Educate and provide support/skill building to clients in budgeting skills to reach their financial goals.

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.The ability to pass the screening required by Federal Human Services Office of Inspector General to assure the employee has not been excluded from working in the Health Care Programs which receive federal funding.Ability to pass a Criminal Background Check.Current driver's license along with a good driving record and valid insurance, if applicable.Required Education and ExperienceMaster's degree in a behavioral science or related field; ORBachelor's degree in a behavioral science or related field and proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; ORHas completed 30 semester hours or 45 quarter hours in behavioral science or a related field and:Has proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields; ORHas 2,000 hours of experience in the delivery of services to people with mental illness; ORHas 2,000 hours of experience in the delivery of services to people with traumatic brain injury or developmental disabilities

and

completes training on mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects; ORIs fluent in the non-English language of the cultural group to which 50% of the practitioner's clients belong; ORHas a high school diploma and 4,000 hours of experience in the delivery of services to people with:Mental illness; ORTraumatic brain injury or developmental disabilities

and

completes training on mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects; ORHas a high school diploma and 2,000 hours of experience in the delivery of services to people with:Mental illness, emotional disturbance, or substance use disorder,

and

receives clinical supervision from a mental health professional once a week until 4,000 hours of experience is met; ORTraumatic brain injury or developmental disabilities

and

completes training on mental illness, mental health de-escalation techniques, co-occurring mental illness and substance abuse, and psychotropic medications and side effects

and
