Summary of Major Responsibilities:

The Case Manager reports directly to the Executive Director. He/she will assist clients to access needed medical, educational, social, prevocational, vocational, rehabilitative, or other community services. They will focus on coordination of SUD care, integration around primary care especially for clients with a chronic substance use disorder, and/or co-occurring conditions, and interaction with the criminal justice system, if needed.Duties:Support and implement the overall purpose, philosophy, values, and direction of House of Metamorphosis, Inc.Actively participate on the program’s treatment team and attend case conferences to exchange client updates, identify client needs, and collaborate on treatment strategies.Case management duties include:Utilizing information gathered at intake and through ongoing assessment, collaborate with the client and the treatment team to determine needs and create individualized case management service plans for clients.Comprehensive assessment and periodic reassessment of individual needs to determine the need for the continuation of case management services; assist with revision of client treatment plan when necessary.Actively help clients to apply for and maintain health and public benefits i.e., Medi-Cal, Housing, etc.Link clients to community resources and services (i.e., transportation, food and clothing assistance, family planning services, legal assistance, educational resources and job training, etc.).Provide direct services to clients, including one-on-one meetings, to facilitate referrals in support of clients’ treatment plan goals (i.e., psycho-social-services; HIV and TB testing and counseling; assistance obtaining driver’s license, birth certificates, and Social Security cards; medication-assisted treatment; supportive housing, insurance).Provide recovery support and care coordination to enhance treatment completion including engagement, outreach, clean and sober socialization, life skills workshops, and other groups as required.Coordinate activities with State, County, and community entities i.e. Probation, Superior Courts, Housing Providers, etc. Work closely with Intake to verify probation and court.Coordinate care with physical health, community health clinics and providers, and mental health providers.Engage clients in his/her treatment and recovery by identifying and developing additional supports such as linkages to self-help groups, peer support activities, and recovery monitoring and coaching.Maintain regular progress/case management notes, records, data entry, and other documentation as required, that meets DMC standards. Experience-Knowledge of Evidence Based Practices; CBT, MI, RP, TIC, PSY-ED.Facilitate necessary transitions with staff in ASAM levels of care (i.e., initiating referrals to the next level of care, coordinating with and forwarding necessary documentation to the accepting treatment agency, etc.).Advocate for clients across various systems to ensure appropriate and timely linkages to physical and mental health care, transportation, and retention in primary care services. Mental Health must be coordinated within HOM mental health team.Communicate face-to-face or by phone with Health and Human Services (HHS) social workers, Behavioral Health Services (BHS) workers, Probation Officers, CWS, Housing Providers and other providers who are critical to the success of clients. Work as a team to identify needs, develop strategies, and identify resources to support clients in achieving abstinence, self-sufficiency, physical and mental health, and financial, employment, and spiritual goals.Identify resources, conduct outreach, develop relationships with community partners, and maintain up-to-date resource directories to ensure clients have access to needed services.Facilitate warm hand-offs where appropriate.Providing or arranging transportation for clients to and from medically necessary treatment.Promote wellness and role-model self-care in professional activities in support of the

tenets.Attend all required meetings and trainings and maintain up-to-date certifications to ensure service delivery is ethical, culturally competent, gender-responsive, trauma-informed, and evidence-based.Adhere to all personnel requirements such as the agency code of conduct, and policies detailed in the employee handbook including, but not limited to dress code, timely reporting, and certification/training requirements.Rotating Weekend Schedule.Qualifications, Skills, Knowledge, and Experience:

Must be a certified/registered alcohol and drug counselor in compliance with all DHCS standards licensed and/or licensed eligible California Board of Behavioral Sciences. Preferably Master Degree in Counseling, Social Work or Psychology. Past experience should demonstrate MFT or Master Degree level of behavioral health experience; proven ability to work across multiple systems such as Probation, CWS, criminal justice, etc.; and strong knowledge of and relationships with community resources. Must have a clean driving record, with a minimum of two years driving experience. Skills include excellent written and verbal communication, ability to manage multiple tasks in a fast-paced environment, strong documentation, and proven success as a team player. Must comply with all agency and contract requirements related to trainings, public health, certifications, reporting, etc. Must be able to pass a drug test, background clearance, and provide a negative TB test. Individuals with bilingual proficiency preferred.Benefits:
