If you would like to be a part of our future team, please apply now!GENERAL SUMMARY:The rehab case manager is responsible for the coordination of services required for an organized multidisciplinary team approach to patient care. This individual will provide more streamlined and coordinated care for patients, assuring quality cost-efficient care for the acute rehab patient population.These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Insight Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers and guests.Responsible for the coordination of services required for an organized multidisciplinary team approach to patient care.Assures quality cost-efficient care for the rehab patient population.Works collaboratively with interdisciplinary teams, department members and other healthcare providers both internal and external to the organization to improve patient care through effective utilization and monitoring of healthcare resources.Assist patient and/or designee in making appropriate referrals depending on patient level of care/needs in the Community.Responsible for therapeutic treatment plans on assigned patients and assisting in process when necessary/requested.Maintains necessary Case Management documentation based on referral source's needs and as applicable on a case-by-case basis.Monitors post-discharge services as applicable and provides needed referrals.Participates actively in team and department meetings.Serves on hospital committees as designated.

Qualifications:

MSW degree with LSW required.Excellent written and verbal communication skills.Licensure to practice in the State of Illinois.Two (2) years experience preferred, preferably in rehabilitation setting and/or discharge planning.Insight Employees are required to be vaccinated for COVID-19 as a condition of employment, subject to accommodation for medical or sincerely held religious beliefs.
