POSITION SUMMARY

The Behavioral Health APRN provides comprehensive mental health care services to clients across the lifespan, including assessment, diagnosis, treatment, and ongoing management of psychiatric disorders. This leadership role is responsible for building the infrastructure required to implement an integrated healthcare model for consumers of Hoyleton's behavioral health services. Involves working collaboratively with a multidisciplinary team to develop and implement individualized treatment plans, promote mental health education, and ensure high-quality client care. This position requires advanced knowledge in the fields of medicine and community mental health.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Patient Assessment and Diagnosis:

Conduct thorough psychiatric assessments through interviews, observations, and diagnostic testing.

Evaluate clients' mental and physical health, considering medical history, family history, and psychosocial factors.

Diagnose mental health conditions based on the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) criteria.

Treatment Planning and Management:

Develop individualized treatment plans that may include psychotherapy, psychopharmacology, and other therapeutic interventions.

Prescribe, administer, and monitor psychiatric medications.

Provide medical direction to Registered Nurse and Medical Assistant staff as needed for client care.

Patient Education and Support:

Educate clients and their families about mental health conditions, treatment options, and strategies for managing symptoms.

Promote mental health wellness and prevention strategies.

Provide crisis intervention and support during psychiatric emergencies.

Collaboration and Coordination:

Participate in the development of process and practice protocols for the integrated care team.

Train team members and other Hoyleton staff as required to support screening, assessment and treatment intervention activities of the integrated care team.

Collaborate with team members, psychiatrists, psychologists, social workers, and other healthcare professionals to ensure comprehensive care.

Consult with integrated care team members and other members of the Behavioral Health Dept. regarding clients with complex behavioral health and social issues.

Complete outreach to, and build relationships with primary and specialty care providers to create a network of collaborating health care providers.

Coordinate referrals to other healthcare providers or community resources as necessary.

Participate in case conferences and team meetings to discuss client progress and treatment adjustments.

Documentation and Compliance:

Maintain accurate and timely documentation of client interactions, assessments, treatment plans, and outcomes in electronic health records (EHR).

Ensure compliance with all regulatory requirements, professional standards, and institutional policies.

Participate in quality improvement initiatives to enhance patient care and outcomes.

Professional Development:

Stay current with advancements in psychiatric nursing and mental health treatment through continuing education and professional development.

Participate in research activities and contribute to the advancement of knowledge in the field of behavioral health as available and/or as approved by Hoyleton.

Adheres to and follows all other required duties necessary and assigned by direct supervisor.

EDUCATION AND/OR EXPERIENCE

Required:

Master's or Doctorate degree in Nursing with a focus on psychiatric-mental health.

Required:

Active Illinois Board-Certified APRN license.

Preferred:

certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).

Minimum of 2 years of clinical experience in psychiatric or mental health settings preferred.

Strong knowledge of psychiatric disorders, psychopharmacology, and psychotherapeutic techniques.

Excellent communication, interpersonal, and organizational skills.

Ability to work independently and as part of a multidisciplinary team.

Commitment to person-centered care and continuous improvement.

REQUIREMENTS

Must be at least 21 years of age

Must possess and maintain a valid driver's license

Must possess and maintain proof of vehicle insurance

CARE & TCI

One of the essential job functions of any position within the agency is to align your performance with our overarching philosophy of CARE. All decisions, actions, communication, and performance should align with the six CARE principles (Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed). Not only must you complete the CARE training with satisfactory participation, but you must then put that training into practice in your everyday work.

Every employee must also pass TCI training with a satisfactory grade in written and/or physical testing. All employees must remain current on refresher training that is provided monthly at all locations. You will be tested every 6 months on the physical portion, if applicable to your position. You will be tested annually on the written portion, regardless of your position. If you are not current on your TCI refresher training every six months, you will be required to retake the full course. If you do not pass either test, you may be required to retake the full course. Not only must you complete the TCI training with satisfactory participation, but you must then put that training into practice in your everyday work.
