The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities.The RN, Clinical II, Outpatient provides intermittent visits to the terminally ill and palliative patients and family, providing symptom management and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing, and psychosocial support to provide appropriate care as the patient transitions to the end of life.Essential Functions and Responsibilities of the JobUtilizes the nursing process and practice standards as a framework for the management of patient care.Analyzes the assessment data to determine a nursing diagnosis and appropriate interventions.Demonstrates leadership in the coordination of patient care across the healthcare continuum with progress towards meeting desired patient outcomes.Demonstrates the ability to utilize effective critical thinking skills.Prioritizes essential functions.Demonstrates knowledge of the clinical information system and applies principles to patient documentation.Contributes to an effective and safe working environment.Formulates a teaching plan, based on identified patient learning needs, and evaluates effectiveness of learning; family is included in teaching plan as necessary.Demonstrates knowledge and understanding of patient privacy rights. Maintains confidentiality of all medical, financial and other sensitive material in printed, electronic or verbal form, which may jeopardize the privacy of patients and or employees.Demonstrates the ability to use good customer service skills by managing and acknowledging patient-family-significant other expectations.Demonstrates tact, sensitivity and discretion.Be at work and be on time.Follow company policies, procedures and directives.Interact in a positive and constructive manner.Ability to prioritize and multitask.Minimum RequirementsQualifications/Work Experience:1-2 years relevant experience as a Registered Nurse in an acute care setting. Home Health, Oncology, and/or Hospice experience preferred.Effective problem-solving skills; good interpersonal and communication skills; ability to relate to a variety of health professionals in different settings.EMR experience preferred.Education/Licensure/Certification:Current RN license issued by the California Board of Registered Nursing.BSN preferred.BLS issued by the American Heart Association.Hospice and Palliative Care Certification preferred.Must possess a valid California driver's license and vehicle insurance (California minimum).
