Job SummaryReporting to the Rehabilitation Team Lead, the Physical Therapist is responsible for comprehensive patient management including evaluation, diagnosis, intervention, and patient/family education for development and implementation of individualized care planning in the home setting. Opportunities include working directly in a multidisciplinary fashion, including specialty care groups such as Parkinson's Disease, Covid Recovery, and Post-Acute Stroke care. The ideal candidate demonstrates strong clinical skills in Physical Therapy, with opportunity for clinical growth through education, in-services, and care management groups. This Home Care environment offers family-friendly, flexible hours.Minimum QualificationsEducation:
Bachelors Degree or higher from an accredited physical therapy program.
Experience:
Prior Home Care experience is preferred, or at least one year previous experience in an acute and/or rehab setting.
Licensure and/or Certification:
MA PT license and BLS required within 30 days from hire.
Skills:
Clinical expertise for patient population.Ability to prioritize workload and respond to urgent needs.Communication skills.Assessment skills.Critical thinking skills.Organizational skills.Problem-solving skills.Computer skills.Ability to maintain confidential information.Ability to read, write and communicate in English.Committed to Service Excellence and customer satisfaction.
