Job Summary:La Rabida is currently recruiting for a Certified Diabetes Educator (RN), 32 hours per week, for the Outpatient Diabetes Clinic in Chicago, IL. We are a small children’s acute care hospital focused on treating complex medical conditions, chronic illnesses, developmental disabilities, and abuse and trauma. Our nurses are focused on providing exceptional patient care to children while helping families face unique challenges.

The Certified Diabetes Educator (RN) has a degree in nursing and specialty certification to teach and manage the diabetic patient population. This individual will coordinate the education and care for the patient and family in both in-patient and outpatient settings.

Essential Certified Diabetes Educator Functions:

Utilizes specialized knowledge, skill, and expertise in Diabetes and Pediatric nursing to coordinate the care of an identified population of pediatric patients throughout the healthcare continuum.

Works with the family and health care team to develop a comprehensive plan of care based on a systematic assessment of the physiological, psychosocial, and developmental needs of the patient and family.

Evaluate and modify the plan of care as needed. Regularly communicates changes to the family, healthcare team, and other agencies involved.

Works with the multidisciplinary team and family to develop individualized family education plans and ensure that patient/family education is completed before discharge (inpatients). Provides appropriate education and follow-up (outpatients).

Collaborates with the family and healthcare team to arrange for home care needs, and school-based care, including home nursing, supplies, and referrals to community agencies.

Provides orientation, staff development, and education of staff on topics related to diabetes.

Acts as a clinical role model by providing clinical leadership to nursing staff in the care of the diabetic child.

Develop and maintain appropriate educational materials and programs to meet the needs of the diabetic child and his/her family.

Documents all patient care activities appropriately in the medical record.

Promotes innovations in nursing care practice through membership and/or leadership in professional organizations, participation in conferences, and utilization of current research.

Participates in or leads Quality Management initiatives.

Maintains required job skills, professional competencies, current licensure, certification, and ongoing education required of the job as applicable.

Supports the hospital mission and observes hospital core job values with respect to service excellence standards, work quality, productivity, teamwork, and initiative.

Attends mandatory education sessions: Annual Infection Control, Annual Safety, Disaster, Hazardous Wastes & Material, and others as determined by the hospital and department.

What we need from you:

Education: BSN required

Related Work Experience: Three (3) to Five (5) years of experience in clinical pediatrics.

Licensure/Certification: Current Illinois RN license required, current licensure in Indiana helpful, CDE eligible required. If not currently CDE, must be obtained within 6-12 months of hire.

Specialized Knowledge and Skills: Excellent written and verbal communication skills, excellent interpersonal skills, demonstrated knowledge and expertise in area of specialty, family education and case management.

CPR Required: Only certifications issued by the American Heart Association, or the American Red Cross will be accepted.
