We’re immediately looking for a
Care Coordinator
to join our team! This is for a long-term contract with the
Maui Certified Community Behavioral Health Clinic !
As the Care Coordinator, you will be responsible for assessing patients’ service needs, verifying eligibility, and recommending and scheduling services.

Hours:
Full-time, Monday to Friday, 8:00am-5:30pm with occasional weekends
Location:
Wailuku, Maui
Pay:
$50 an hour
Responsibilities:
Determine patients’ eligibility for clinic services.
Conduct a preliminary assessment for services requested.
Analyze information from medical reports, professional evaluations, interviews, and school reports.
Evaluate adequacy and consistency of information gathered and determine the need for more information.
Recommend and schedule psychological evaluations for individuals, families, and guardians.
Qualifications:
Current Hawaii license to practice as a Clinical Social Worker, Marriage and Family Therapist, or Mental Health Counselor.
Bachelor’s degree with a minimum of two (2) semester credit hours in psychology or social work.
Two (2) years of professional experience providing mental health clinical services to individuals, couples, families, and/or groups with mental illness.
