ResponsibilitiesAs the Director of Care Management you are responsible for planning, directing and evaluating the full scope of care coordination and utilization review services and for providing strategic leadership in assessing, developing and implementing programs and initiatives to form a comprehensive and cohesive model that provides access to care across the continuum of health care services.Essential Duties:Plans, directs and evaluates the programs, systems, operations and resources of the care coordination function.Develops departmental goals.Prepares annual budget and works within the budgetary guidelines.Directs all aspects of patient care coordination within the clinical areas.Works with discharge planning team and external stakeholders to ensure appropriate and indicated hospital utilization, medical necesity, status determination, care coordination, care progression and care transition processes.Directs the development, implementation and evaluation of new/revised policies, procedures, guidelines, training materials and/orgeneral communications in support of the care coordination function.Produces and presents reports on key performance indicators and other benchmarking metrics.Researches, analyzes and assesses the effectiveness of current strategies and practices in meeting expectations and producing desired results.Hires staff, conducts performance evaluations, develops staff and disciplines as necessary.Shift: Monday - Friday 8am - 5pm

QualificationsLicense/Certification:Oregon RN license OR LCSW/MSWExperience:5 years demonstrated, relevant care management experience in an acute care setting as well as 3 years director level experience with direct staff reports.Education:Required Bachelor’s Degree in related discipline; Master’s Degree strongly preferred
