This position also reviews clinical documents including medication records and referral information as part of the hospice intake process as well as providing phone support in the hospice office as needed. This position will also include support for the field nurses when needed, including phones calls, and focused visits, under the direction of a RN and per the plan of care.Essential ResponsibilitiesManages all incoming phone calls for hospice.Under direction of an RN, reviews all incoming referrals and collaborates with clinicians to gather necessary information to determine appropriateness for hospice services. Collaborates with Medical Directors and hospice leadership as needed.Collaborates with RN and other members of the hospice office regarding referrals to ensure timely intakes are scheduled.Provides telephonic triage support to patients, families, caregivers as needed for Hospice field staff and advocates for patient needs through support and communication with hospice IDT.Assists in reviewing and assigning RN case manager and Hospice team to new referrals.Under direction of an RN, makes patient visits as needed for back-up and support.Gathers and reviews medical records and medication list for accuracy.Responsible for documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards according to policies and procedures.Assists with hospice chart reviews, as requested.Orders DME, medications or supply refills through PBM/3rd party vendor, as appropriate and in line with the practice standard.Organizational ResponsibilitiesPerform work in alignment with the organization's mission, vision and values.Support the organization's commitment to equity, diversity and inclusion by fostering a culture of open mindedness, cultural awareness, compassion and respect for all individuals.Strive to meet annual business goals in support of the organization's strategic goals.Adhere to the organization's policies, procedures and other relevant compliance needs.Perform other duties as needed.Experience and/or EducationRequiredGraduate of an accredited LPN training program.Current unencumbered LPN license in the State of Oregon.Minimum 1 year experience in hospice, palliative care/oncology, geriatrics and/or home health.Experience providing triage support for patients.CPR certification at hire or within 6 months in position.PreferredMinimum 2 years' experience in hospice, palliative care/oncology, geriatrics and/or home health.Knowledge, Skills and Abilities RequiredKnowledgeKnowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations.Skills and AbilitiesDemonstrates ability in case management, nursing and counseling in a home-based setting.Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation.Must have general computer skills and a working knowledge of Electronic Medical Records systems, MS office, Outlook, and the internet.Excellent interpersonal skills.Ability to tolerate and manage grief.Ability to work both independently and collaboratively.Ability to work under pressure to meet deadlines.Ability to take initiative and utilize innovative techniques and ingenuity.Ability to interact with various departments throughout the organization.Ability to work flexible hours, if needed.Ability to plan, prioritize and coordinate multiple projects or tasks.Ability to work effectively with diverse individuals and groups.Ability to learn, focus, understand, and evaluate information and determine appropriate actions.Ability to accept direction and feedback, as well as tolerate and manage stress.Ability to see, read, hear, speak clearly, sit, and perform repetitive finger and wrist movement for at least 6 hours/day.Ability to lift, carry, push, pull, bend and/or pinch small objects for at least 3 hours/day.Working ConditionsWork Environment(s): Indoor/Office, Community Facilities/Security, Outdoor Exposure.Member/Patient Facing: No, Telephonic, In Person.Hazards: May include, but not limited to, physical, ergonomic, and biological hazards.Equipment: General office equipment and/or mobile technology.Travel: May include occasional required or optional travel outside of the workplace; the employee's personal vehicle, local transit or other means of transportation may be used.Schedule: Occasionally required to work more than 8 hours per day.Candidates of color are strongly encouraged to apply.
