Job Description - Nurse Practitioner-Family Medicine-Physician Practice Full Time (3271932)PURPOSE OF YOUR JOB DESCRIPTION:The purpose is to support the Family Medicine Physician Practice and to provide coverage when the providers are out of the office.DELEGATION OF AUTHORITY:They will report to the providers they work with and will utilize our nurses and MA’s in the offices as well.JOB FUNCTIONS:Every effort has been made to make your job description as complete as possible. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or is a logical assignment to the position.PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES:Diagnose and Treat acute and chronic IllnessPerform age-appropriate history and physicalOrder and interpret diagnostic and therapeutic tests relative to patient’s age-specific needsPrescribe appropriate pharmacologic and non-pharmacologic treatments to patient’s age specific needMonitor the effectiveness of treatmentsProvide education of acute and chronic medical conditionsManage Chronic Medical ConditionsReconcile medication listFacilitate appointments to specialistProvide thorough exams and evaluations of patient’s health statusFacilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling)Preventive Medicine:Keep track of and refer patient to appropriate preventive medical screenings (e.g. Mammograms and Colonoscopies)Counsel and reinforce patients in healthy lifestyle habitsEDUCATION REQUIREMENTS:• Primary Care NP certified in MassachusettsEXPERIENCE:• 3 years experience in a Primary Care setting• Proficient Computer Skills (MS Word)• EMR experience required• Ability to document in electronic health record completely and accurately• Knowledge of clinical reminders and how to use them appropriately• Good communication skills, both spoken and written• Willing and able to work in a team environment• Able to change with the ever evolving healthcare field• Master Degree, ACLS, and PALS preferredEqual Opportunity Employer:Martha’s Vineyard Hospital and Windemere Nursing & Rehabilitation Center are Equal Opportunity Employers.
