Health Services Director - Memory Care

Frontier Senior Living is now seeking an outstanding

Health Services Director (RN, LVN, or LPN)

to manage and oversee the Health Services Department for

Dogwood Trails Assisted Living and Memory Care

community located in

Palestine, Texas . Demonstrated success as a leader in similar settings is required.

The

Health Services Director (RN, LVN, or LPN)

works closely with the Executive Director and Resident Care Coordinator to ensure the quality care of all residents. This position requires an understanding and commitment to the principles and values of the community and must demonstrate flexibility with residents who have complex situations. Responsibilities include assessment and service planning to ensure resident care needs are evaluated, as well as providing training and oversight of the Medication Techs and Caregivers.

Qualifications:

Demonstrated ability to appropriately delegate responsibility with necessary authority while maintaining overall accountability.

Ability to identify problems in a timely manner and develop effective resolutions.

Primary Duties and Responsibilities:

Implement interim service plans in partnership with the Resident Care Coordinator as care needs change.

Provide oversight and documentation for third-party providers for coordination of care.

Conduct routine audits among other departments as directed by the Executive Director.

Implement systems and protocols per company standards and compliance with state, federal, and industry regulations.

Maintain compliance practices per regulations and stay updated on changes.

Familiarize with the service plan program and its reports.

Order incontinent supplies and durable medical equipment as needed.

Ensure communication and documentation from home health, hospice, and ancillary care providers.

Complete MAR audits and monitor the medication room for organization and cleanliness.

Assist with tracking and recording incident reports.

Work as manager of the day as directed by Executive Director.

Maintain a monthly budget for the Health Services department.

Track health services staff training requirements.

Full management responsibilities for healthcare staff including hiring, training, and performance reviews.

Complete delegation training, assessments, and documentation per state guidelines.

Ensure compliance with clinical budget in staffing and supplies.

On-call for clinical support and staffing coverage as needed.

Other tasks as assigned.

Other Requirements:

Must have appropriate nursing licensure (RN, LVN, or LPN) active and in good standing.

Proficiency in English preferred.

Maintain CPR certification.

Experience in clinical nursing practice or long-term care administration preferred; new grads welcome!

Must possess a current state-specific driver's license and appropriate driving record.

Must be able to lift and carry 50 lbs.
