Job Title:
RN Care Manager
Duties:
Travel to members’ homes and facilities to conduct face-to-face needs assessments and follow up with telephonic contact, adhering to guidelines and protocols.
Assess, plan, coordinate, and evaluate care for members with chronic and complex needs in various settings.
Coordinate and address physical, behavioral, social, and psychosocial needs across the continuum of care, ensuring safety and cost-effectiveness.
Facilitate care authorizations, transitions between care settings, and continuity of services.
Educate members and caregivers on healthcare needs, benefits, and long-term care options.
Develop and implement care plans with members or caregivers, addressing specific needs and goals.
Identify and facilitate referrals to specialized programs, including Disease Management and Behavioral Health.
Job Type: Contract
Pay: $35.00 - $40.00 per hour
Expected hours: 40 per week

Experience:
Intensive Case Management/Discharge Planning: 5 years (Required)
Assessments, developing Service Plans and Care Plans: 3 years (Preferred)
PCP’s, Occupational Therapists, Behavioral Health: 2 years (Preferred)
DME Equipment: 2 years (Preferred)
License/Certification:
Certified Case Manager (Preferred)
RN License issued by DE state (Required)
