Recognizing and valuing exceptional talent, we're eager to discuss the exciting details of this bonus during our interview process.General SummaryThe Clinical Social Worker works as integral member of the Population Health Team and assigned ambulatory care practice, working closely with the clinical and administrative staff to enhance the delivery of patient care services along the continuum of care. The Social Worker identifies high-risk psychosocial factors of patients, helps primary care staff understand the influence of those factors upon the course of medical care and assumes care coordination responsibility of the patients. In some cases, and as appropriate, the Clinical Social Worker may provide direct treatment/intervention to patients and families. In other cases, the Clinical Social Worker may work with the treating outpatient mental health clinicians, within and outside of our system, helping to ensure that treatment is focused and effective.The Clinical Social Worker works as part of the Integrated Care Management Program (iCMP) embedded within the practice. iCMP is a Population Health initiative for high-risk patients who are medically, psychiatrically, and socially complex.Upon referral from a team member, the Clinical Social Worker assesses and evaluates a patient's psycho-social needs and/or serves as the lead care coordinator to manage patients who have social work needs.The Key Functions of the Clinical Social Worker are:Comprehensive assessment and case managementPatient navigationHealth PromotionCrisis ManagementSelf-management educationTransitional carePatient and family supportLiaison to community servicesShort-term counselingAdvanced care planningTeam consultationQualifications:Current LICSW/LCSW in MAMinimum of Masters-level degree program in social workMinimum of 2 years clinical experience.
