We are seeking a compassionate and dedicated Registered Nurse to join our outpatient primary care team. The successful candidate will play a crucial role in supporting our physicians, ensuring smooth daily operations, and delivering high-quality care to our patients.

Main duties:

Physician Support:

Assist physicians with patient care, including preparing patients for exams, administering medications, and providing follow-up care instructions.In-Basket Management:

Efficiently manage the electronic health record (EHR) in-basket, including responding to patient inquiries, processing test results, and addressing other clinical tasks.Prescription Refills:

Handle prescription refill requests, ensuring timely and accurate communication with both patients and pharmacies.Coordination of Care:

Collaborate with healthcare providers, patients, and their families to coordinate care plans, referrals, and follow-up appointments.Medicare Annual Wellness Visits:

Conduct Medicare Annual Wellness Visits, assessing patient needs, updating health records, and developing personalized care plans.Patient Education:

Provide education to patients and families on health conditions, medications, and lifestyle changes to support overall health and well-being.Compliance:

Ensure compliance with all regulatory requirements and best practices in patient care and documentation.

Schedule:

Monday - Friday; 8:00 am - 4:30 pm

The Registered Nurse Outpatient provides safe/therapeutic care in a holistic and systematic way in a physician practice or other outpatient setting. Incorporates differences into the provision of care while integrating knowledge, skills, and experiences to meet the needs of patients and families to include patient and family education. Achieves desired patient outcomes by using an integrated approach. Incorporates data and information to improve care and enhances outcomes while ensuring improvements in practice settings. Provides direction and guidance to others regarding practice while serving as a resource, preceptor, and mentor.

Job Responsibilities:

Performs essential duties necessary to clinic or non-acute operations to include taking vitals, stocking exam rooms, giving vaccines, blood draws, etc.Formulates plans of care based on trends of similar patients including patient education and discharge planning. Identifies variances from expected outcomes. Evaluates patient outcomes and makes revisions to plans of care.Collaborates with patients/families to identify realistic desired outcomes. Adapts standards and guidelines for individual patients. Tailors and prioritizes caring practice to individual needs including cultural, ethical, and spiritual needs. Participates in the planning of routine transitional healthcare needs (i.e. treatment options, patient placement options, end-of-life options). Responds to changes in patients' needs and provides holistic caring.Makes independent clinical decisions based on knowledge drawn from education and experience.Actively advocates for patients' rights and identifies potential conflicts.Ensures efficiency in the delivery of care. Determines immediate priorities and offers solutions for problems. Works with the team to achieve maximum productivity.Adapts planned educational programs and information to individual patients and families by modifying teaching strategies or content.Documents all patient care activities as per documentation standards. Assists the healthcare team with the documentation process.Participates in work groups and/or committees. Keeps informed of initiatives and incorporates the outcomes of team or committee work into practice.Acts as a clinical resource and serves as a mentor to healthcare team members while displaying leadership behaviors.Performs other duties as assigned.

Requirements:

Certification:

Basic Life SupportLicensure:

Licensed as a Registered Nurse in the state of employment or an active multi-state Registered Nurse licenseExperience:

None required.

Preferred:

2 years of ambulatory RN experience or ED experience.Education:

Bachelor's Degree in Nursing or Associate's Degree. If RN has an Associate's Degree, must complete BSN within 5 years of start date.
