We are hiring for an RN Case Manager to join our Hospice Team in Santa Barbara. This is a full-time position (20-25 visits per week, Monday - Friday 8 am to 5 pm) and we pay $81 per routine and $141 per eval visits.

What you will do:Have flexible hours and working close to home (reduce your commute!)Complete initial and ongoing comprehensive assessments of the patient’s needsProvide service and/or treatments requiring substantial and specialized nursing skillsInitiate appropriate preventative and rehabilitative nursing proceduresCounsel, support and instruct the patient, family and caregiver in meeting nursing and related needsWho you are:Graduate of an approved school of professional nursing.Current, active license in the state of California Board of Nursing.Current CPR, health clearance, TB clearance.
