Whether you have many years of experience or are in the early stages of your career, you can find a rewarding career at Grady!SUMMARYThe Nurse Navigator will be responsible for enhancing the patient experience in Breast Imaging by providing navigation services from screening mammograms to diagnostic services and onto the treatment process by providing individualized assistance to patients and families. Helps patients overcome barriers within and outside the healthcare system. Works closely with the Breast Imaging radiologists to enable timely access to breast screening and diagnostic services. Collaborates with the cancer center for transition of patients who need services in the cancer center.Job ResponsibilitiesResponsible for collaborating with the Breast Imaging radiologists and members of interdisciplinary team to serve as the primary point of contact for patients and families in Breast Imaging.Responsible for coordinating and managing patient care in Breast Imaging through scheduling, supporting, educating, advocating, providing a link to research, addressing barriers to care access, and providing indirect patient care.Responsible for monitoring and coordinating scheduling, patient tracking, procedure education and follow-up care to ensure completion of recommended care.Provide image-guided biopsy results when appropriate. Hand off to Breast Specialist and Oncology RN Navigator as needed.Responsible for helping to increase access to Breast Imaging by working with community navigators to arrange scheduling of needed mammograms.Works in collaboration with patient scheduling, radiologists, social workers and others to determine and address barriers to care.Coordinates and manages data reporting and tracking in systems such as EPIC, MagView and other systems to demonstrate reduction of barriers and case management.QUALIFICATIONSBachelor's Degree in Nursing is required.Master's Degree in Nursing is preferred.Three (3) or more years of direct nursing experience and demonstrated clinical competence.Case management experience is preferred.Current licensure as a Registered Nurse in the State of Georgia is required.Basic Life Support (BLS) certification is required.Demonstrate competence in clinical specialty.Utilize advanced clinical thinking and analytical skills that contribute to patient adherence of treatment protocols and symptom management.Competent in use of Electronic Medical Records.Equal Opportunity Employer-Minorities/Females/Veterans/Individuals with Disabilities/Sexual Orientation/Gender Identity.
