Responsibilities:The Central Community Health Partnership (CCHP) is a collaboration of five premier human service organizations in Central Massachusetts that has been selected to provide Behavioral Health (BH) Community Partner services. We help individuals with complex needs navigate access to services and assist in finding the right combination of services, as it can be an overwhelming task. The vision is integrated care that ensures that every member receives the right care at the right time – making good health and wellness a reality, not a dream.

The BH Senior Clinical Care Manager will provide and oversee integrated care coordination and care management to individuals enrolled in the Central Community Health Partnership’s Behavioral Health Services (BH) Community Partners Program. The Senior Clinical Care Manager will supervise a team of Care Coordinators who provide integrated care management in collaboration with individuals served, community partners, medical and behavioral health providers.

The schedule for this role is Monday-Friday 830a-430p.

Other Key Responsibilities:

Participate in the development and implementation of program goals, objectives, and work plan tasks that are directed toward accomplishing the Mission and Vision of the CCHP.

Support the growth of the BH clinical team by providing leadership and expertise to building clinical focus of program.

Provide clinical consultation and recommendations to care coordinators, providers, and/or others as appropriate. Senior Clinical Care Manager will lead consultation with medical director as needed.

Work collaboratively with the CCHP/BH team, and medical and behavioral health providers to coordinate clinical information for care management and coordination, care transitions, medication reconciliation and health and wellness coaching.

Collaborate with Individuals Served, Care Coordinators, Medical and Behavioral Providers, and other members of the team in the implementation and monitoring of Person-Centered Treatment Plan (PCTP).

Review and oversee the assessment and person-centered care plans for clinical appropriateness, appropriate service referral/utilization and integration across all health domains.

Facilitate weekly staff meeting.

Qualifications:BH Senior Clinical Care Manager(s) shall have at minimum:

At least two years of relevant professional experience and hold the following qualifications: A licensed independent clinical social worker (LICSW), A licensed mental health counselor (LMHC), A licensed marriage and family therapist (LMFT), A licensed alcohol and drug counselor I (LADC I), A licensed psychologist; A licensed certified social worker (LCSW); A licensed rehabilitation counselor; or A registered occupational therapist.

Certified Care Management License preferred, but not required.
