Job Summary and QualificationsYour goal as a mental health professional is to create discharge plans that address patients' psychological, social, cultural, and financial needs. This helps them recover and maintain their overall well-being. This role works as a mediator between clinical teams, patients, families, and next-level care providers to ensure seamless and comprehensive care.
What will you do in this Role:
Conduct individual, group, and family psychoeducational groups.
Contribute to treatment plan development and attend treatment planning meetings and reviews on an ongoing basis.
Assess environment and patient’s safety status and report/respond to actual and potential safety issues or changes in the patient's condition.
Identify risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
Complete relevant assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope.
Collaborate with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
Conduct post-discharge follow-up as indicated.
What qualifications you will need:
Basic Cardiac Life Support must be obtained within 30 days of employment start date.
Nonviolent Crisis Intervention must be obtained within 30 days of employment start date.
Bachelor's Degree.
