Job Description - Pharmacy Ambulatory Patient Navigator/ Part Time 20 hours (3305313)Brigham and Women’s Anticoagulation Management Service (AMS) is a pharmacist-run clinic that provides full anticoagulation management services for all oral anticoagulants including warfarin (Coumadin, Jantoven), dabigatran (Pradaxa), rivaroxaban (Xarelto), apixaban (Eliquis), and edoxaban (Savaysa). BWH AMS has been recognized as a Center of Excellence by the Anticoagulation Forum. Patients are referred to our service by a Brigham and Women’s/Faulkner Hospital attending physician, usually the patient’s primary care provider or a specialist such as a cardiologist or hematologist for management of anticoagulation therapy. BWH AMS is a service dedicated to caring for patients who require anticoagulation. Pharmacists will provide full anticoagulation management including, but not limited to, prescribing anticoagulant medications and their required laboratory monitoring tests, while working closely with the referring doctor under a Collaborative Practice Agreement.We provide the following services:Selection of most appropriate oral anticoagulant therapy based on patient specific variables (including direct-oral anticoagulants and warfarin)Thorough education on anticoagulation based on national patient safety goalsAssistance with getting your medication filled at the pharmacyOrdering and following labs based on patient specific factorsManagement of anticoagulants around procedures or surgeriesAssessment and tools to improve adherence24/7 emergency pager for triaging bleeding and clotting concernsPatient Navigator will report to Ambulatory Pharmacy Manager and be part of a core team that works closely together to provide exceptional patient care.Principal Duties and ResponsibilitiesPatient navigation assistantContacts patients enrolled in BWH AMSCoordinate patient enrollment including obtaining patient medical history and laboratory data.Facilitate patient access to information provided by the program.Communicate recommendations to patients as provided by the CDTM pharmacistAssist and help coordinate program developmentHelp with patient correspondence and schedulingHelp with any administrative tasks as neededTeaching and EducationEducate patientsWork with the CDTM pharmacist to provide information and education to patientsOther duties as requested.Minimum QualificationsCollege degree preferred. Especially well-suited for this position might include individuals with background/major in Public Health, Health Education, Communications, Psychology, Clinical/Health Research. Master’s in Public Health potentially beneficial but not required. Similarly, individuals with pharmacy background and training may also be well suited for this role given relevant issues related to drug therapy. Prior experience with clinical research or customer service beneficial. Individuals interested in future positions prior to medical school, public health also potentially well suited for this role.Skills and Abilities RequiredExcellent organizational skills given coordination of multidisciplinary team, various logistical components, data acquisition.Strong communication skills, especially regarding support for patients and their questions, issues; empathy, ability to learn, adapt to program approaches.Comfortability with telephone-based workflowEstablished ability to work with, coordinate teams.Computer skills: working knowledge, familiarity with computer, software including use of email, Excel.Personal abilities/attributes: Positive attitude, creative problem solving, hardworking, flexible, energetic, cognizant of patient confidentiality, appropriate professional demeanor, especially in interacting with physician offices.Fluency in Spanish or other languages are desirable; and being proficient in utilizing interpreter servicesOccasionally may require hours outside normal working hours, such as SaturdaysComfortable expressing ideas and feedbackPrior experience as a Navigator Assistant or equivalent 1 year experienceAble to work independentlyWorking ConditionsWork will be remote, must be able to attend on-site training and meetings as needed at Assembly Row.Brigham and Women’s Hospital is an Equal Opportunity Employer.
