In this vital role, you’ll work closely with our Center Director and lead an 11-discipline Interdisciplinary Team (IDT) to ensure our participants receive the highest quality care. What will success look like for our Care Team Specialist
Work closely with our interdisciplinary team to truly understand and honor each participant's goals, values, and preferences, ensuring that our care plans are person-centered and reflect What Matters Most (WMM) to them.Embrace and apply the principles of Patient Priorities Care (PPC) and Goal Concordant Care, guiding our approach to delivering compassionate, individualized care.Lead and facilitate Care Team Meetings, morning huddles, and care planning discussions, ensuring that all important details are accurately captured in our Electronic Medical Record (EMR).Create and maintain efficient workflows for care planning, documentation, and compliance, making sure everything aligns with regulatory requirements while supporting our high standards of care.Serve as a go-to expert in our EMR system, providing ongoing support and guidance to your team members, helping them feel confident and capable.Keep everything on track by monitoring and ensuring the timely completion of assessments, care plans, service determination requests, and other IDT-related tasks.Step in as a supportive backup for the Scheduling Coordinator, ensuring assessments are completed within the required 30-day timeframe.Partner with our Quality Improvement and Compliance team to audit care plans and ensure we’re always meeting regulatory requirements with excellence.Offer coaching, support, and nurturing a culture of accountability, positivity, and continuous improvement.Support team members in delivering compassionate care wherever it’s needed, meeting participants where they are—physically, emotionally, and spiritually.Other duties and responsibilities as needed to support the growth of the IDT team.What does an ideal team member look like?
Passion and mission orientation for serving high-risk seniors and frail older adults.Thrives in a fast-paced environment and a culture that values collaborative, team-based approach.Brings a patient-centered mindset, focusing on what matters most to the patient and enabling patients to reach their care goals.Bachelor's Degree in a relevant field (e.g., nursing, social work, healthcare administration), Masters in Social Work or Healthcare Administration strongly preferred.Minimum of two years of experience providing direct care for participants, preferably in a PACE or similar setting.At least one year of experience in a PACE organization or value-based care organization.Strong leadership and communication skills, with a proven ability to hold others accountable and foster collaboration among team members.Compliance-focused mindset and familiarity with regulatory requirements related to PACE.Proficiency in Microsoft Word, Excel, and PowerPoint.Experience using Electronic Medical Records (EMR) systems.Have one of the following licenses: LVN, RN, LCSW (preferred but not required).Other pertinent information
This position is 100% onsite, and supporting our Greater Los Angeles Market.Schedule is Monday - Friday 8am - 5pm.This position will require to be in our on-call rotation.
$80,500 - $100,500 a year

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role.
