You will serve as the liaison for residents, their loved ones and staff as it relates to customized resident service plans. Through expert leadership, coaching and mentorship you will develop and foster strong clinical and care teams focused on delivering consistently exceptional care and services.ESSENTIAL FUNCTIONSResponsible for overall compliance of clinical services & programs including accuracy of medication administrationWorks collaboratively and facilitates communication with physician offices, ancillary services, responsible parties and internal care teams to deliver quality resident careWorks collaboratively to complete timely resident assessments and to develop & initiate the Care & Service PlanSupports and assists in day-to-day care and clinical needs as neededConducts interviewing & hiring, evaluates employee performance, provides coaching and employee training, and delegates assignments for all direct reports
