Job Details

Monday through Friday Hospice RN Case Manager covering EITHER LaPorte area or Plymouth area. Seeing patients in facilities and homes.

What you will be doing:

  • Patient Care Coordination: Manages and coordinates all aspects of a patient's end-of-life care, including symptom management, pain relief, and emotional support, working closely with doctors, caregivers, and family members.
  • Assessment and Planning: Conducts comprehensive assessments of patients' physical, emotional, and psychological needs to develop and adjust personalized care plans, ensuring comfort and quality of life.
  • Education and Support: Provides education and guidance to patients and their families about the dying process, care techniques, and available resources, while offering emotional and psychological support to help them cope.
  • Documentation and Compliance: Maintains detailed records of patient care, treatment plans, and interactions, ensuring all documentation meets regulatory requirements and organizational standards for quality and compliance.

Skills you need:

  • **Indiana RN license, or ability to obtain
