Job Details

We are seeking a compassionate, dedicated, and experienced Hospice Registered Nurse for a permanent on-call position, working a 7-on-7-off schedule. As an integral part of our interdisciplinary team, you will have the opportunity to provide end-of-life care to patients in their homes, ensuring they are comfortable, at peace, and treated with dignity and respect during their final days. This position requires an individual who can demonstrate empathy, patience, and an exceptional understanding of palliative care.

Responsibilities:

As a Hospice Registered Nurse, you will be responsible for:

  1. Providing holistic, end-of-life care to patients in their homes, focusing on managing symptoms, alleviating pain, and providing comfort measures.
  2. Performing patient assessments, developing individualized care plans, and regularly updating these plans to reflect changes in the patient's condition.
  3. Administering prescribed medications, monitoring their effectiveness, and managing any side effects.
  4. Assisting patients and their families in understanding the patient's condition, prognosis, and the processes of death and dying.
  5. Making pronouncements of death, providing emotional support to the bereaved, and coordinating with the hospice team to ensure appropriate post-mortem care.
  6. Advocating for the patient's wishes, ensuring these are respected and incorporated into their care plan.
  7. Documenting patient care accurately and in a timely manner using the Oasis system.
  8. Collaborating with the interdisciplinary team, including physicians, social workers, chaplains, and bereavement coordinators, to provide comprehensive care.
  9. Participating in on-call duties, responding promptly and professionally to patient emergencies.

Qualifications:

The successful candidate will have the following qualifications:

  1. Active Registered Nurse license in good standing.
  2. Minimum of 2 years of nursing experience, preferably in home health, hospice, or palliative care settings.
  3. Proficiency in using the Oasis system for patient documentation.
  4. Extensive knowledge and understanding of the processes of death, dying, and pronouncement.
  5. Demonstrated experience in patient advocacy and end-of-life care.
  6. Proficient in medication administration, with a strong understanding of pain and symptom management.
  7. Exceptional communication skills, with the ability to provide education and support to patients and their families.
  8. Strong critical thinking, problem-solving, and decision-making skills.
  9. Ability to work independently, manage time effectively, and respond to urgent situations promptly.
  10. Must have a valid driver's license, reliable transportation, and be willing to travel to patients' homes.
