This includes activities that support and enhance stabilization and well-being. In addition, upon completion of batterer’s intervention certification, this position is required to facilitate a weekly batterer’s intervention group.This position interviews and assesses applicants for services, composing and implementing service plans of action with SMART and manageable goals. All clients will have an assessment, service plan with goals & objectives, and progress notes. The Case Manager II will facilitate daily/weekly recovery groups, workshops/psychoeducation groups, and coordinate other workshop activities that will support, promote, and enhance clients’ self-sufficiency and growth mindset.The principal duties are performed in an office environment and in the field and/or communities as needed. This position is expected to collaborate and table at community events in the Tenderloin and throughout San Francisco’s marginalized communities. This position is expected to be onsite, Monday through Friday 9:30am to 6:00pm. There will be occasional need to work evenings, weekends, and early mornings.Essential Duties and ResponsibilitiesCoordinate and manage a caseload of at least 15 clients with a professional, trauma-informed lens, client-centered, efficient, and equitable framework to help clients achieve their best stability, wellness, and autonomy.Facilitate and complete client assessments, service plans, timely progress notes, determine case closure, and enter all documentation into database systems (ONE System and Salesforce) daily.Identify participant strengths and barriers to stability; assist participants in reducing barriers or challenges and connect them to internal and external resources and services.Proactively assess barriers to stability for individuals experiencing homelessness to determine the best course of action and support service navigation efforts to ensure service needs are addressed.Conduct/facilitate/coordinate workshops/groups daily that are inclusive, reflective, and guide participants through the stages of change.Support service navigation for seniors to ensure social connections and resources.Adhere to professional boundaries and standards; understand and follow basic ethics, values, and principles of regulatory practices. Adhere to all laws and requirements regarding HIPAA, confidentiality, child protective services, contractual agreements, etc.Meet professional obligations through efficient work habits such as collaborating with co-workers, meeting deadlines, honoring schedules, attending and actively participating in staff and individual meetings, community meetings, supervision, case conferences, client conferences, and in-service trainings.In collaboration with supervisor guidance, complete a minimum of 15 hours of approved professional training and skill development each year.Minimum QualificationsMinimum of three (3) years of experience in a recovery-oriented field serving high-risk populations; or a bachelor's degree in social work/psychology or a related field, plus 2+ years’ experience working with socially and/or economically marginalized, multiply diagnosed populations.Experience performing case management and group facilitation services; ability to conduct interviews and assess client needs.Strong verbal and written communication skills; ability to connect with the client base.Proficiency in spelling, grammar, and writing is needed to draft detailed instructions and case service plans.Demonstrated knowledge of principles and techniques of assessment, intervention strategies, counseling, and harm reduction practice.Ability to successfully work independently and collaboratively within a team.Ability to multi-task projects and meet specific timelines; strong attention to detail, particularly in documentation in case files and updating the client database.Works well under pressure and manages stress effectively in a demanding and fast-paced environment.Strong conflict resolution skills; ability to de-escalate oneself and others who may have difficulty managing their own behavior due to emotional, mental, or physical illness and/or substance use.Knowledge of SF resources, services, and benefits programs, including services and programs for families with children, is helpful.Ability to demonstrate proficiency with basic computer programs including Microsoft Word, Outlook, and Excel; familiarity with the ONE System and Salesforce is strongly preferred.Certificates, Licenses and/or RegistrationsCurrent Peer Specialist or SUD Certification, or willingness to secure within 90 days from the date of hire.Duluth Facilitator Certification and the Domestic Violence Certification are required; Glide is willing to coordinate access to complete this training.$28.80 - $31.25 an hourThis role is covered by the collective bargaining agreement.Physical Requirements:Reasonable accommodations can be made to enable individuals with disabilities to perform the essential functions:Ability to work on a computer and see details of objects at close range.Ability to hear within normal range, and communicate effectively (in person, telephone or Zoom).Finger dexterity and the ability to use all standard office equipment.Sit or stand comfortably; and the ability to navigate throughout office spaces (via elevator or stairs).At GLIDE, we ensure competitive and fair compensation by utilizing data from seven comprehensive pay surveys, including Gallagher, and analyzing over 450 nonprofits in our sector.
