Schedule: Typically 5x8's Days 0800-1630On-Call/Weekends: Every 3rd weekend, on-call once weekly as neededRate is all-inclusive
Patient caseload: 25-30 patients for case managers, 6-7 visits daily for non-case managers
Years of Experience: Minimum 1 year in home health
Length of assignment: 13 or 26 weeks available
Additional information: New EMR HomeCare HomeBase implementation occurring, will receive training on new EMR. Do not pay mileage for travelers, any overtime must be pre-approved, timely communication and documentation required.
Required certifications/license: RN licensure with Ohio Board of Nursing, driver’s license with proof of insurance, BLSAbout LeaderStat
