TravelThe Minimum Data Set Coordinator is a professional registered nurse responsible for the completion of astandardized assessment tool call the MDS to evaluate the health and well-being of residents in Medicareor Medicaid certified beds. This position may be required to manage patient care and cover for urgentstaffing needs. The MDS Coordinator assumes responsibility for administrative duties as delegated by theDirector of Nursing including disciplinary action. They will assure compliance with all thepolicies/procedures of MyMichigan Health and follow Occupational Safety and Health Administration(OSHA) guidelines, Clinical Laboratory Improvement Amendments (CLIA) regulations, and all appropriateregulatory agencies. They will coordinate with and train any new employees, students and float staff asrequired. The MDS Nurse will provide patient education, assist with specialty care modalities, symptommanagement, be available for emergencies, monitor for compliance and the effects of care managementwith high-risk patients, educate on prescriptions and teach the patient and family the reasons for themedication and side effects with discharge planning efforts, support the coordination and ensure continuityof care with other providers.Submission Requirements

2 years of Charge/Supervisor experience - RequiredLong term care experience - RequiredResponsible and accountable for scheduling, completion, accuracy, validation and timeliness of the MDSand care planning process. Reviews medical records and resident care plans for completeness and compliance with State and Federalregulations and standards of practice. Communicates with medical records, billing, and appropriate individuals regarding insurance and codingissues when identified. Maintains the resident roster and census.Completes the assessment upon admission, quarterly, annually, according to skilled services (PDPM)schedule and annually.Demonstrates strong assessment and resident evaluation skills.Coordinated the development of individualized nursing care plans. Supervises nursing actions towardmeeting the goals established in the plan of care including, but not limited to, physical, psychological,social, rehabilitative, educational, and discharge planning needs of the individual. Supervises direct nursing care based on health care practitioner orders supervises medicationadministration, supervises treatments, supervises the delivery of care in accordance with the plan of care. Assists with any resident / family / staff education needed to meet identified learning needs.Understands necessary computer functions: MDS software and Center for Medicaid and Medicare Services(CMS) transmission of the MDS, Michigan Care Improvement Registry (MCIR) Practice Management,Internet Quality Improvement and Evaluation system (iQIES)Demonstrates critical thinking for patient management. Maintains a high level of confidentiality and ensures patient’s rights in accordance with proper proceduresand in compliance with HIPAA (Health Insurance Portability Accountability Act). Profile Requirements:Work HistorySkills Checklist, current to 1 year, pertinent to the specialty2 Supervisory Reference Forms, taken within the last two yearsLicensure & Certifications (MI State License/Lara & BLS - AHA)Candidates' Permanent Address (Facility will not local candidates within 100 mi radius)
