Submission Requirements: - Perm employees cannot return to CAMC as a traveler for 2 Years after last day worked perm - Flu Shot: Please DO NOT SEND ANYONE WHO REFUSES FLU SHOT WITHOUT MEDICAL PROOF. CAMC requires all personnel take flu vaccine unless they have allergy documented by an allergist or physician record. CAMC FLU SEASON IS OCTOBER 1st - MARCH 31st. NOT REQUIRED OUTSIDE OF THAT WINDOW. - Covid Vaccine NOT required - -ONE PDF MUST CONTAIN: Candidate profile (work history), Specialty specific skills checklist, Min. 2 Mgr/Charge References (within past 2yrs.), RN Lic. verification (NURSYs pulled within 2 days of sub date – clean lic. required – hits submitted for case-by-case basis approval), Driver's License, SSN.
