Doctor of Dental Medicine (DMD)
or
Doctor of Dental Surgery (DDS)
from an accredited institution.Active
State of Illinois dental license
and malpractice insurance.Ability to thrive in a
team environment
and maintain strong communication with team members.Licensed to practice dentistry in the State of Illinois.Our Locations:
