Overview

As the Manager of Population Health at Cone Health under The Value Institute, you will play a pivotal role in advancing our commitment to improving the health outcomes of the communities we serve. You will be responsible for direct oversight of the teams to optimize population health management initiatives. This role requires a blend of clinical expertise and leadership to drive measurable improvements in health outcomes and cost efficiencies.Talent Pool: Corporate Services/ProfessionalResponsibilities

Team Leadership: Provide leadership and guidance to team members, fostering a positive and productive work environment. Oversee the recruitment, training, and development of team members to ensure high performance and career growth. Monitor team performance, provide regular feedback, and conduct performance evaluations to recognize achievements and address areas needing improvement. Encourage and support the professional growth and development of team members through training, mentorship, and opportunities for advancement.Population Health Design: Oversight of population health teams which strive to improve community health care delivery, and the patient experience using clinical and financial performance information, proven methodologies, and successful partnerships. Design initiatives to improve the management of populations across the network that demonstrate measurable improvements in quality, service, and cost reduction.Continuous Improvement: Develop strategies for continuous improvement, implementing best practices and innovative approaches to enhance efficiency and effectiveness. Identify and address challenges and obstacles that arise, proactively seeking solutions to ensure the team's success. Coordinate the use of analytics, process improvement, etc. to evaluate and improve the effectiveness of population health programs and interventions. Maintain responsibility for the growth, development and oversight of population health and clinical integration initiatives across the continuum of care.Stakeholder Management: Build and maintain relationships with key stakeholders, including clients, partners, and senior management, to ensure alignment and support for team objectives. Leads initiatives with cross-functional teams and senior leaders to achieve targeted strategic performance goals.

Qualifications

EDUCATION:

Required: Bachelor?s degree in nursing, Healthcare Administration, Public Health, Pharmacy, or related field.EXPERIENCE:

Required: 5 years of experience in healthcare management, with a focus on population health management or care coordination.3 years? experience in value-based care, ACO?s, or clinically integrated networks.LICENSURE/CERTIFICATION/REGISTRY/LISTING:

Required: Clinical background and active licensure (e.g., RN, NP, PA, CP)
