POSITION SUMMARY:The clinical pharmacist in this position is responsible for all pharmaceutical services in assigned areas. As a member of the multidisciplinary healthcare team, the clinical pharmacist promotes an environment of safe, rational, effective and efficient medication use while providing services to meet the needs of the patient. This role also performs other pharmacy services as appropriate.

RESPONSIBILITIES:* Reviews orders for appropriateness and completes final verification.* Assesses the effectiveness, safety, and affordability of patient medications. Work collaboratively with the healthcare team to develop and optimize patient medication therapy and provide pharmacotherapy consultations as requested in practice area.* Provides drug information on medication use, dosage, side effects and composition to physicians, nurses, and patients/families. Monitors inpatient and outpatient profiles for drug interactions, food-drug interactions, and duplication of therapy.* Participates in transitions of care and assists with issues involving patient access to medications.* Participates in quality improvement activities such as floor inspections, medication use evaluations, and documentation of interventions. Has knowledge of best practices and how own area integrates with others; demonstrates awareness of the industry, including regulatory requirements, evolving customer demands, and the factors that differentiate the organization in the market.* Supports contributions to pharmacy and medical literature. Participate in research where applicable. Maintain knowledge in area of expertise. Provide education to pharmacists, pharmacy residents, nurses and providers. Precept pharmacy students, residents and fellows.* Participates in the development, implementation and ongoing review of Epic order entry and set build.* Attends hospital committee meetings as needed.* Participates in continuing education programs, in-services, and workshops to maintain current knowledge and enhance professional growth.* Performs other duties and special projects as required.

This is a full-time position working weekdays on a regular pattern and every 4th weekend.

WORK EXPERIENCE:* At least 1 year experience in hospital pharmacy

*Completion of a pharmacy practice residency with pediatric experience (preferred)

*Pediatric experience in clinical specialty areas (preferred)

EDUCATION:*Bachelor degree in pharmacy (BS Pharm) or Doctorate of Pharmacy (PharmD) from an accredited school of pharmacy- Required

* Completion of a pediatric clinical pharmacy residency- preferred

LICENSES AND CERTIFICATIONS:*Current Texas Pharmacist license-required

*Pharmacist Preceptor license- Required within 90 days- required* Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course- Required within 90 Days-required

* 20 hour ACPE accredited IV training course- Required within 90 days- required* Board of Pharmacy Specialties Certification-preferred
