We are so excited to announce that BoldAge PACE has two new locations opening in 2024! We will open new facilities in Owensboro, KY and Evansville, IN! These locations will be known as Heartland PACE.
BoldAge delivers innovative, creative solutions for PACE (Program of All-Inclusive Care of the Elderly). Our model of care enables older adults to live a meaningful, independent life at home with grace and indignity.
We meet the evolving needs of older adults by providing innovative disciplinary solutions that consider both their physical and mental well-being. Our owner operated model is aimed at expanding the ability for older adults to live happily and healthfully in their own homes for as long as possible.
We are seeking a Physician who is dedicated to making a difference in people's lives by providing comprehensive, compassionate care to join the BoldAge PACE Team!
Position Summary:
Under the supervision of the Medical Director, provides primary care and continuous medical coverage services to PACE participants. Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide service in accordance with PACE Program standards. Collaborates with any Nurse Practitioners employed by Beacon of LIFE in the provision of assessments, reassessments, care planning and the provision of primary care.
Basic Qualifications:
Education/Training/Certifications:• MD or DO - Compact License required• Current State physician license - Compact License required• Board Certified - Internal Medicine or Family Practice• State CDS Certification and DEA Registration• Board Certified - Geriatrics preferred.• Current driver's license and proof of auto insurance
Skill(s):• Thorough knowledge of current concepts, theories and practices related to home and community-based care for the elderly and disabled adults.• Working knowledge of health care delivery and financing systems including Medicaid, Medicare, waiver programs, prospective and capitated systems, public health programs and HMO/Managed Care.• Thorough knowledge of physical, mental and social needs of frail older adults.• Effective oral and written communication skills• Ability to lead and work within the interdisciplinary setting.• Strong organizational skills.• Able to manage changing priorities per needs of PACE Program and the agency.• Legible handwriting.• Working knowledge of PACE regulations• Valuable team member; works well with others.• Manages participants comfortably in all settings.• Medical Director seeks out and has complete confidence in recommendations.• Valuable member of committees.• Excellent clinical skills.• Provides valuable improvements to participants and PACE Program through the review and analysis of utilization data.
Experience:
Minimum of 2 years working with the frail or elderly
