Join Our Mission at Cascadia Health!
At Cascadia Health, we deliver whole health care - integrated mental health and addiction services, primary care, and housing - to promote hope and support the well-being of the communities we serve. We envision a community where everyone benefits from whole health care, experiences well-being, and has a self-directed, connected life.
About the Role:
We are excited to announce an opening for a Licensed Medical Provider (LMP) to join our Intensive Case Management (ICM) Team. The LMP will provide psychiatric assessments and ongoing medication management to consumers receiving ICM services at Cascadia clinical programs, while supporting a Trauma Informed Care environment. The ICM team serves individuals 18 years and older who have been diagnosed with a serious mental illness. This multi-disciplinary team provides intensive, flexible community support to improve the health status of clients by reducing psychiatric symptoms and substance use, enhancing community engagement, functioning, and quality of life. Given the client level of care needs, the ICM team maintains a smaller staff-to-client ratio.
As part of the ICM team, LMP services may include consultations to external partner providers/agencies, formal and informal case consultations to internal and external care team members, coordination with primary care and other external providers, and support to family members or other support systems as needed. While most work will be performed in the clinic setting, occasional community-based work is expected.
