Our ultimate goal is to restore a maximal level of function and help prevent re-injury.Responsibilities:Perform initial and ongoing PT evaluations that evaluate mobility, strength, and current level of function according to APTA standards of practice.Develop and implement an individualized plan of care for each patient based upon evaluations, goals, and attainable outcomes.Coordinate care with an interdisciplinary team to restore patients to their best level of function, increase independence, and improve quality of life.Reinforce patient accountability to ensure consistency and compliance with their treatment program.Maintain medical documentation and policies/procedures; communicate to patients and caregivers.Create a post-treatment plan to continue their improved quality of life; recommend assistive equipment as needed.Maintain professional and technical knowledge in orthopedics, sports, pelvic floor, pain management and/or vestibular rehabilitation by attending educational workshops, reviewing professional publications, and participate in monthly continued education staff presentations.Qualifications:Valid Colorado physical therapy license.Prefer 2+ years’ experience in orthopedics, sports, pelvic floor, pain management /or vestibular.Degree in Physical Therapy from an accredited PT program .High level of empathy, a positive outlook, and the ability to motivate patients to push their limits in a safe and effective way.Understands responsible use of facility equipment, best practices for use, and preventative maintenance procedures.(Jazz HR)
