OPWDD has authorized Maximus to conduct these required assessments for individuals ages 17 and younger, using the Child and Adolescent Needs and Strengths (CANS). The CANS is a person-centered assessment tool that is very effective at helping OPWDD to learn more about the strengths and needs of children and adolescents with intellectual and/or developmental disabilities.

The CANS provides important information to assist with care planning and connecting individuals with appropriate services.Our dedicated team is proud to be part of the OPWDD program.

We are doing great things every day!Position OverviewWe are looking for a well-qualified and passionate health care professional for a new position with the CANS program.

Operation Support Coordinators will be responsible for outreaching to individuals and their caregivers/primary supports, and Care Coordinators to provide education, answer questions and schedule CANS assessments.

The incumbent will be responsible for providing excellent customer service to the population we serve.This is a limited-service position, anticipated to extend for the contract duration of 18 months.

The position may be shortened or lengthened based on business needs.If you meet position qualifications, this is a great opportunity to gain valuable experience and grow your career.Position SummaryConducts outreach to OPWDD Care Coordination Organizations to obtain pertinent consumer information and identify and inform necessary parties of impending assessment appointment.Conducts timely outreach and meets productivity targets related to scheduling assessment appointments with all appropriate individuals identified and documents all outreach activities for tracking and reporting purposes.Ensures consumer needs are met when scheduling assessment appointments and the most appropriate resource is assigned to complete the assessment.Ensures the scheduling process and workflow runs efficiently and meets all program objectives and quickly identifies areas for improvement or correction.Assists in coordinating last minute scheduling changes to ensure assessment appointments are completed and quickly communicates updates to appropriate parties.Respond to inquiries from all stakeholders including individuals, families and CCO agencies, and provide education about the Child and Adolescent Needs and Strengths Assessment (CANS).Provides administrative support to the Management staff.What You Will NeedHigh School diploma or equivalent.1-3 years of experience doing community outreach or conducting one-on-one presentations; experience working or interacting with individuals with intellectual or developmental disabilitiesBilingual preferred, but not requiredPreferred experience 1-3 years of experience in a customer service environment and familiarity with population served; experience with MAXeb, Choices and OPWDD servicesJob Summary

Education and Experience Requirements
