And so do we!Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home.You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family educationYou will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patientsYou will review patient insurance information and coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregiversCurrent NYS RN (Registered Nurse) license and registration requiredBachelor's degree in nursing preferred; Associate's degree requiredMinimum of one year acute medical-surgical nursing experience requiredCertified Home Health Agency (CHHA) experience preferredBilingual English and Spanish or Mandarin or Cantonese preferred
