Upperline Health Plus is seeking an experienced Medical Assistant to support a team of clinicians in our
Zephyrhills, FL clinic delivering complex plans of care in accordance with the patient's health status and overall goals and values.
In addition to clinical support, this individual will be providing care coordination, including all telephonic and back-office support required by the team of clinicians.
In this role, you get to act as a patient's guide to a complex and confusing health care system. You will provide critical support activities to coordinate personalized care delivery for Upperline Plus patients and patients' families.
Ideal candidates will have experience identifying and coordinating the needs of patients with multiple chronic conditions as well as supporting clinicians to help them operate as efficiently as possible. Experience with an older adult / geriatric population is strongly preferred. Successful candidates should be excited and energized by the prospect of working collaboratively as part of an innovative, patient-focused care team. WHAT YOU'LL DO
Conduct health assessments and complete visits with Upperline Plus patients being seen in our Podiatry ClinicsComplete vitals and required documentation in clinic EMR (AthenaHealth)Provide routine foot care for at-risk patients with Diabetes and/or other chronic illnessFacilitate and support virtual provider encounters in the clinicTelephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative careCare coordination activities as assigned by Upperline Plus Nurse Practitioner including, but not limited to, medical records requests, order management, appointment scheduling, managing patient care referrals, etc.Visit patients in their homes to complete routine foot exams to meet the needs of our Upperline Plus patients as neededIdentifying community resources and assisting Upperline's patients and families in accessing those resourcesParticipation in weekly interdisciplinary team meetingsOther related duties as deemed necessary by Upperline HealthQUALIFICATIONS OF THE SENIOR MEDICAL ASSISTANT
A minimum of 2 years' experience as a medical assistant providing direct patient care is requiredExperience in an outpatient clinic setting is strongly preferredClinical care coordination or care management experience is idealPrevious experience assisting or providing care to the adult - geriatric patient population is preferredPrior wound care and foot care experience is a plusBilingual English/Spanish is helpfulPrevious experience working in an EMR system - ideally, AthenaHealthDatabase management skills and proficiency using Word, Excel and other Microsoft Office products is necessaryMust have reliable transportation, valid driver's license and minimum state required liability auto insuranceExtremely strong customer-service orientation and work ethicExceptional organizational skills and ability to multi-taskSelf-starter with an entrepreneurial spirit and demonstrated problem solving skillsExcellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful
COMPENSATION
Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
