Primary days/hours are: Monday- Wednesday 8am-4:30pm. This is a flex schedule, days can change based on coverage needs. No weekends.
Basic Function : This position is responsible for coordination and oversight of evaluations in the Emergency Department and admissions (at HVB only), discharge planning activities, coordinating referrals and aftercare services for patients.
Qualifications:Required:
Graduate of an accredited school of nursing with current Pennsylvania license. Successful completion of applicable clearances as outlined in Human Resources policy HR-106 within 90 days of commencing employment. CPR certification. Must have a minimum of 3 years current experience in an inpatient psychiatric setting. Knowledge/ability to assess and utilize community resources.
Preferred:Previous experience with psychiatric case management. ANA certification as a psychiatric nurse.
